When your pet has an appointment to come in to AMC, it will save you some time at check-in if you complete your paperwork ahead of time. We offer many forms in e-form that will be emailed directly to us.

Some of our forms have a lot of information. Depending on your pet's needs, you may have some serious decisions to make. We know that you may have questions for the nurse or doctor before you can complete the forms. That's fine! We want to help you understand your options so you can make informed decisions for your pet. Feel free to call us with your questions or ask to speak with a nurse or doctor at the time of your pet's admission. We are happy to help!

Our fees are adjusted quarterly or more frequently if needed to reflect our changing costs of doing business and our policies are frequently updated to reflect current medical research and the best medical practices. Growth Removal Form (.pdf)	The Growth Removal Form has a drawing of the pet for you to mark the location(s) of growths, lumps, bumps or rashes that will be removed or treated. This form is added to the appropriate Procedure Authorization form when appropriate.
